A care coordination nonprofit serving Angelina and Nacogdoches counties needed one clear front door for families looking for healthcare, food, housing, and transportation help.
What we built. A guided resource site with a "Not sure where to start?" helper, a care request intake, a community health worker academy with registration, an events calendar, donations, and a private staff sign in.
